The Health Information Services department is responsible for medical records, coding, tumor registry, dictation, transcription, and release of medical information. The medical record primarily serves the patient; however, it has many additional values to the medical care team and CTRC administration. The most obvious use of the medical record is to document and review patient care encounters. Patient care providers rely on the medical record as the principal means of communication and information exchange regarding patients under their care. Each patient has one legal medical record of which there may be multiple volumes. The record is maintained and retained by Health Information Services.
